Sports Physicals:
Before you can tryout for any sport, you must have a valid physical on file with the JFK Nurses. A physical is valid for 13 months after it is done (for example, a physical completed on July 11, 2005 is valid until August 11, 2006) and must be renewed annually. Be sure you have the correct form for your doctor to sign. Forms will be mailed to all students in Grades 6 - 12 but, in case one does not find its way to you, they are available from any school nurse or the Enfield Public Schools Athletic Department. Do not leave this task until September --- tryouts will proceed without you if the proper form is not on file! Please mail or fax the completed form to the school nurses as soon as possible. For further information and to download forms, please go to Athletics or to Health Services.
Scoliosis Screenings:
All students must have scoliosis screening done during this school year. Screenings are completed during the P.E. classes and referrals will be mailed out.† Any student not in P.E. will be screened in the nurses office before May 15th.
Secondary Schools Medical Update:
During the school year, please keep the nurse updated on your child's current medical issues. We also appreciate having current phone numbers available in order to contact you.
Medications:
Students are not allowed to carry any type of medication (except asthma inhalers or an Epi-pen at the secondary level). Students who need to carry their inhalers or have medications in the nurse’s office need to have the proper forms on file with the nurse. For further information and to download forms, please go to Health Services.
New Immunization Requirements for Grade 7
The Connecticut State Legislature is expected to pass additional immunization requirements for entrance into seventh (7th) grade effective for the start of the 2009 school year. The new immunizations are as follows:
For entrance into 7th grade (in addition to current mandates):
Tdap (1 dose)
Varicella 2nd dose
Mumps 2nd dose
Rubella 2nd dose
Please be sure your child’s physician updates this vaccine(s) before the start of school.
Please note students will not be allowed to start school if written proof of the vaccine(s) is not provided to the nurse.

HEALTH ROOM PRACTICES
A student is ALWAYS sent home from school for the following conditions:
*
Temperature of 100 or higher
*
active vomiting or diarrhea
*
suspected contagious diseases : conjunctivitis(pink eye), chicken pox, strep throat, scarlet fever, head lice, impetigo, scabies, ring worm, undiagnosed suspicious rash
If a student complains of a headache or stomach ache (with none of the above), they will rest for 15 minutes and return to class.
If a student complains of headache or stomachache because he/she did not eat breakfast, a cereal bar will be offered.
If a student returns several times or requests the phone, a call will be made to a parent or guardian.
If a student does not feel well enough to participate in Physical Education (with none of the above), the nurse can sign the student out ONCE. The student will make-up the class after school when feeling better. If the student does not participate in other PE classes, a doctorís note is required for a medical excuse.
If a student has an injury or disability that impacts the school day (i.e. cast, crutches, brace or limited walking or carrying etc.) a doctorís note is required with specific limitations and length of disabilities.
If a student has a physical injury that prevents participation in PE, the doctorís note serves as a medical excuse and does not affect the grade.
If a student has an injury, he/she is to report to the nursesí office on the first day back. The nursesí issue the passes, set up the buddy, etc. All injured students must be on record in the nursesí office in case of emergency.
As always, in middle school, the above guidelines are subject to change with every situation.
